Q: Is my insurance in-network for BOA?
A: We are an in-network provider for Blue Cross Blue Shield PPO plans (not including Blue Choice). We are out of network but will submit claims for other out-of-network PPO providers including: United Healthcare, Aetna, and Cigna.

Q: How do I get to the clinic?
A: We are located at 3100 Dundee Road, Suite 704, in Northbrook, Illinois 60062. Our clinic is located between Pfingsten Road and Landwehr Road. You can easily access our clinic from the Edens Expressway by exiting West Dundee Road. If you are having difficulties finding the clinic, please call the front desk at 847.663.1020 ext 110.
